Assessments FAQs
Find quick answers to the most common questions about conducting Assessments in Libryo Assess.
How do I create an assessment?
Admins and Members can create assessments. Go to the entity, click + New Assessment, enter a name, choose the assessment protocol, and click Create Assessment. If you are not sure which protocol to use, ask your ERM project manager or Organisational Admin.
How do I answer questions and record observations?
Open the assessment, expand a section, select a question, enter your response and click Save, then click Next to continue. Use + Add Observation to record observations against a question. Admins and Members can answer questions and record observations. Changes are saved automatically, and you can click the X at the top right to exit. See How to Complete an Assessment.
What is Applicability Screening?
Applicability Screening decides which sections and questions apply to an assessment before you answer them in detail. Open the assessment, select Applicability Screening, answer Yes or No for each item, and click Save.
How do I change an assessment's status or mark it as done?
- To mark an assessment as done, select the Mark As Done checkbox on the Assessment Summary page.
- For other statuses, open the assessment, click the Settings icon at the top right, click Manage Assessment, and choose the status you need.
Admins and Members can mark an assessment as done or inapplicable. Every status change is logged at the bottom of the Manage Assessment panel.
What do Confidential, Archive and View-Only do?
These options are available to Admins only. Confidential hides the assessment from Members, Contributors and Certifiers who are not on the assessment team. Archive hides it from Members, Contributors and Certifiers. View-Only locks the assessment so it cannot be edited.
How do I group assessments?
When you create an assessment, add it to an existing assessment group or create a new one. You can also create a group from the Assessment Group page in the entity's assessment list. An assessment can belong to only one group.
How do I add or remove assessment team members?
Open the assessment and click Assessment Team. To add someone, click Add Member, select the users and confirm. Only users who already have access to the entity can be added. To remove someone, click the delete icon next to their name and confirm.
Can I see sections that have been marked as inapplicable?
Yes. Use the View Inapplicable Sections toggle to show them in the sidebar. This is available to Admins, Members and Certifiers.
Can I recover a deleted assessment?
Yes. Deleted assessments can be restored through the Activity Log. Only Admins can delete assessments. Contact your ERM project manager or Organisational Admin if you need one restored.
My assessment template or form won't load and shows "The requested resource was not found". What should I check?
- Check whether the problem affects every template or only some, and which sites.
- Ask your ERM project manager or Organisational Admin whether the protocol has been retired.
- Try on both desktop and mobile to see whether it fails on both.
- If it continues, email support.assess@erm.com with the template and site names, and mention any inspection date coming up so we know how time-sensitive it is.
